Marco Materazzi's last-minute goal gave Inter a draw at the Olimpico, preventing Roma from notching up a 12th straight win.
This is traditionally an entertaining encounter - with a total of 15 goals scored in the last three editions - and the fact that Roma were celebrating a record 11th consecutive victory only added spice to the fixture. Inter were also determined to get a result here so that the fourth-placed Giallorossi wouldn't close the gap on them in third.
It was Roma's second game without Francesco Totti, who is out for three months with a fractured ankle, while Vincenzo Montella and the suspended Alberto Aquilani were also sidelined, so Amantino Mancini was the makeshift striker and 19-year-old youth team midfielder Aleandro Rosi given his big chance.
Inter had to do without Adriano and ex-Olimpico hero Walter Samuel, who were both serving bans, while Oba Martins started from the bench after his midweek trip home on compassionate leave. There were several ex-Lazio stars representing Inter, including Coach Roberto Mancini and winger Cesar Aparecido, who only made the bitter move in January.
Alvaro Recoba, who has been struggling with injury of late, had the first chance after four minutes and Doni smothered his long-range snapshot at the second attempt.
Roma went even closer soon after, as Mancini got a touch to Taddei's free kick and Francesco Toldo - standing in while Julio Cesar recovers from an eye infection - made a reaction save at the near post.
The opener came moments later, as Mancini's sudden change of pace left Marco Materazzi behind and he nutmegged Pierre Wome with his pass for the totally unmarked Taddei to sidefoot home from six yards. The Nerazzurri back line was static in this occasion and allowed the two Brazilians to carve them up. As with the derby last week, Totti was on a special bench near the Curva Sud and the players all ran over to celebrate with their captain.
Materazzi made up for his error by charging down Simone Perrotta's shot on the counter-attack, then Ivan Cordoba's penalty appeals for a Philippe Mexes shirt tug were waved away.
Inter were sluggish and Coach Mancini opted to make a substitution after just 34 minutes, replacing Kily Gonzalez with Luis Figo. The Argentine did not even cross glances with Mancio as he walked back towards the bench. The Nerazzurri were giving the ball away easily and wanted too much time on the ball, so they had difficulty creating team moves for this reason.
The visitors created a great chance just before the break, as Cruz sprung the offside trap and set up a splendid Esteban Cambiasso volley into the ground that forced Doni into a save at full stretch.
From the resulting corner kick, Doni was alert to Recoba's low cross-shot at the near post.
Soon after, there was another opportunity as Inter finally stepped up the tempo, with a dangerous Figo cross cleared at the last second. There was tension going into half-time, as Daniele De Rossi claimed he had been elbowed by Cruz.
After the restart, Recoba's left-foot drive was deflected off target by De Rossi for a corner kick, then Materazzi hooked an ambitious effort well wide.
Roma's counter-attack was dangerous with Olivier Dacourt winning back the ball, but his pass was just short of Mancini on the edge of the area.
Recoba's hopeful shot was straight at Doni, then Stankovic unleashed a powerful snapshot from outside the box that skimmed the crossbar with the goalkeeper stranded.
Roma had another fantastic opportunity on the break, as Mexes took advantage of an error and immediately passed across to Mancini for a first touch and strike that was inches over the bar.
Inter maintained pressure for long periods, but the home defence held strong. Christian Panucci attempted an audacious backheel flick on Simone Perrotta's free kick, but couldn't quite make contact and Toldo gathered.
Mancini made another change, replacing Cruz with Martins, but Materazzi was booked for two dangerous tackles in the space of five seconds.
From a well-worked corner kick, Cristian Chivu's deflected strike was held by Toldo.
Inter's best chance of the game so far came on 67 minutes, as Doni punched a Recoba corner kick up into the air and Martins looped his header over the goalkeeper, but De Rossi was ready for a decisive goalline clearance to save the Giallorossi's blushes.
Doni hurt his shoulder in that incident and was moving gingerly, as soon after he fumbled the ball again under pressure from Stankovic.
Materazzi intercepted a dangerous cross for Mancini inside the area, then Cesar replaced Pizarro, moving Stankovic to central midfield for the final 15 minutes.
A Mancini free kick shaved the upright and the simmering tension in this match spilled out when Recoba got into a scuffle with Panucci and Dacourt. The Frenchman went in hard on Martins and, in a bid to get to Dacourt, Recoba pulled Panucci's hair.
Moments later, Recoba's shot was charged down by Chivu inside the area, then a Stankovic close-range effort was cleared by the defence as far as Wome, whose powerful strike was pushed round the upright by Doni.
A Recoba volley was also charged down as Inter piled on the pressure, but at the other end Panucci's free header on a Taddei set-piece was over the bar.
Perrotta sprinted into the area on his own, but hesitated and allowed the defence to clear.
In the final minute, Materazzi rose to nod in a Figo corner kick from six yards, getting in front of Panucci for his free header.
