Rangers conceded a goal early in either half to fall behind after the first leg of their Champions League qualifier.
Brazilian striker Wagner Love was given room to head the opener for the Russian champions after just four minutes.
But Nacho Novo scored his first competitive goal for Rangers when he shot home after a 37th minute scramble.
Czech midfielder Jiri Jarosik put the Moscow team back in front when he cut through the Rangers defence with ease to strike in the 46th minute.
With the first attack of the game, Croatia international Ivica Olic powered his way down the left and fired in a cross that flicked up off Zura Khizanishvili's outstretched leg and fell perfectly for Love to head home from close range.
Soon after, Stefan Klos had to look sharp to keep out a 30-yard free-kick from Jarosik that had taken a deflection off Fernando Ricksen.
Rangers looked uncertain at the back and Love was given room when Gregory Vignal and then Jean-Alain Boumsong failed to clear but Klos pushed away the striker's shot.
Rangers were enjoying more possession but did nothing to trouble the home defence until an equaliser arrived eight minutes before the interval.
Dado Prso took advantage of a loss of control by Vasili Berezutski and slipped the ball to strike partner Novo but the Spaniard's first touch was heavy as keeper Akinfeev came to meet him.
The pair collided as defenders arrived in the box to help, with Prso also wading in and eventually the ball broke kindly for Novo to make amends by slotting into net on the turn from ten yards.
CSKA were ahead again just seconds into the second half when Jarosik ran on to a throw-in, shrugged off Peter Lovenkrands, cut inside Maurice Ross and thundered a powerful shot past Klos from an acute angle.
Love got in behind Ross after an hour but his low shot from 18 yards flashed wide of the target.
A curling free kick from Rolan Gusev then had Klos at full stretch to push the ball round for a corner.
Alex McLeish's side lacked composure in midfield and seldom worked the ball anywhere near the home side's penalty area.
But CSKA were also bereft of imagination in the last third of the pitch and appeared content with a one-goal lead.
Rangers have two league matches before the return leg at Ibrox on 25 August and will hope to be a lot sharper in attack by then.
